Job summary

Mindoula is seeking an experienced Clinical Partnership Manager for its Collaborative Care Program (CCP). The role focuses on activating physicians within contracted practices to refer patients to CCP, with monthly referral quotas and ongoing provider engagement across accounts, practices, and providers.

Travel up to 50% is expected as part of developing strategic partnerships. The CPM will partner with senior leadership to ensure post-implementation provider satisfaction, training, and ongoing

Qualifications

• Bachelor’s degree required in a healthcare-related field or business.
• Three+ years in sales or account management with healthcare provider referrals preferred.
• Willingness to travel up to 50% and manage multiple accounts.
• Experience presenting to groups in person and virtually.
• Proven track record growing accounts and driving referrals.

Responsibilities

• Grow and nurture partnerships within the Collaborative Care Program at account, practice, and provider levels.
• Activate physicians to refer patients to CCP and meet monthly referral goals.
• Coordinate regular meetings with accounts to review enrollment, opportunities, concerns, and achievements.
• Create and review referral performance reports with territory accounts.
• Communicate with providers to ensure referral intake and address ongoing needs.
