Colorado Department of Human Services is seeking a remote Clinical Care Coordinator to provide therapeutic interventions and comprehensive care coordination for Civil and Forensic clients. The position involves home-based work with on-site travel for meetings primarily in the Denver metro area, with arrangements subject to change.

Qualifications include Colorado licensure (LCSW/LPC/LMFT/LAC) and at least one year of licensed clinical experience in behavioral/mental health care.

Qualifications

• Current, valid Colorado licensure as LCSW, LPC, LMFT, or LAC.
• One year of licensed experience in direct care behavioral/mental health treatment.
• Preferred: 2+ years of behavioral health experience.
• Bilingual in Spanish strongly preferred.
• Located within 45 miles of 81501.

Responsabilités

• Provide therapeutic interventions and clinical care coordination for patients in jail and hospital settings.
• Individualizes psychosocial therapy approaches to meet patient needs.
• Coordinate with Care Coordination Program and stakeholders to develop case plans for care and placement.
• Maintain treatment records and ensure communication with courts, providers, and care teams.
