Job summary

Care Options for Kids is seeking a Client Success Associate to manage end-to-end referral and admission processes for private duty nursing services in the home. You will liaise between referral sources, patients, families, clinical teams, and payers to ensure timely, accurate intake and service activation.

The role requires coordinating investigations of eligibility, benefits, and authorizations, and maintaining consistent communication with all stakeholders while ensuring compliance with

Qualifications

• Two years of healthcare intake/admissions/care coordination experience or related field preferred.
• Knowledge of insurance verification, authorization processes, Medicaid, and health plans related to PDN services preferred.
• Proficiency with EMR systems and MS Office (Outlook/Excel) required.

Responsibilities

• Receive and manage referrals from hospitals, physicians, and managed care organizations.
• Coordinate all new patient intakes from referral through admission and service activation.
• Review referral docs for completeness and enter data into the EMR.
