Job summary

Pacific Health Group is seeking a CHW Program Manager to lead CalAIM Community Health Workers delivering whole-person care for Medi-Cal members in San Diego County, CA. You will ensure high-quality service, operational efficiency, and strict adherence to billing and documentation standards.

Responsibilities include supervising CHWs, maintaining billing compliance, documenting care plans, and coordinating with RNs and community partners to improve client outcomes.

Qualifications

• Bachelor’s degree in Public Health, Social Work, Healthcare Administration, or a related field.
• 3–5 years in community health, Medi-Cal services, or social work; 2+ years in supervision.
• Strong CalAIM policies knowledge, social determinants of health, chronic disease management, and local San Diego County resources.
• Valid California Driver’s License (Class C) with an operable vehicle; CHW certification preferred.

Responsibilities

• Oversee and mentor a team of CalAIM CHWs delivering outreach, health navigation, and advocacy.
• Ensure CalAIM billing compliance, CPT coding accuracy, and service unit limits.
• Audit Plans of Care, progress updates, and SD OH assessments across portals.
• Coordinate onboarding and continuing education including Motivational Interviewing and advocacy training.
• Collaborate with RNs, clinicians, and local partners to coordinate care.
