Job summary
Your Health is seeking a dedicated Community Health Worker to support patients across the [LOCATION] area. The CHW links patients with healthcare services, performs home and ALF visits, and ensures timely delivery of medications and DME. Daily travel is required, with 12-hour shifts from 7:00 AM to 7:00 PM.
The role supports care teams and coordinates with providers to improve access and outcomes. Ideal candidates have relevant healthcare credentials and comfort with RPM, telemedicine, and EMR
Qualifications
- Certified Medical Assistant, Paramedic, or licensed nurse in good standing.
- Phlebotomy experience preferred; willingness to train if not experienced.
- Experience in public health or home health preferred.
- Experience with DME delivery or logistics preferred.
- Able to use technology tools for remote monitoring, telemedicine, and EMR.
Responsibilities
- Coordinate and ensure coordinated patient care with care team.
- Communicate effectively via phone, email, Teams, Athena, in person.
- Arrange visits to ALFs and homes; coordinate with PT/OT and providers.
- Deliver medications and review instructions; reconcile meds.
- Coordinate telehealth visits with pharmacists and specialists.
- Set up RPM training and provide ongoing support.
- Deliver DME and train patients on use.
- Perform venipuncture and collect specimens.
