Esrun Health is seeking a Remote Care Manager to oversee a patient panel and deliver care coordination from a home office. You will manage 30–50 patients per month, complete required encounters, and record outcomes with HIPAA-compliant practices.

Ideal candidates have nursing licensure (RN/LPN/LVN) with at least two years of clinical experience, EMR proficiency, and strong communication skills. A reliable computer, high-speed internet, and comfort with Windows/Mac environments are essential.

Qualifications

• RN/LVN/LPN with valid nursing license in current state.
• Minimum 2 years clinical experience in clinic, Med/Surg, Case Management, and/or home health.
• Experience with Electronic Medical Records and Windows-based software.
• Strong time-management and problem-solving skills.
• Reliable high-speed internet and a Windows or Mac computer.
• Excellent verbal, written, and listening skills.

Responsibilities

• Manage patient panel and coordinate care and billing.
• Complete due diligence on assigned patients and ensure billable encounters.
• Attend regular meetings and maintain plan of care for patients.
• Develop care plans and coordinate with doctors and patients.
