Job summary

First 5 Alameda County is seeking a compassionate Health Care Navigator to support Alameda County families. You will respond to calls via the Help Me Grow line, assess needs, and connect families with services that promote child development.

The role involves coordinating referrals for health, developmental, and social needs, documenting progress, and working with community partners to improve service delivery. Remote work is designated, with occasional in-person meetings when required.

Qualifications

• 3 years increasingly responsible experience in child development, social service, or psychology with 2 years related coursework.
• A college degree is not required; relevant work can substitute.
• Bilingual proficiency is strongly preferred.

Responsibilities

• Respond to a high volume of calls and referrals from the Help Me Grow line to support child development.
• Discuss developmental and family needs; create child-centered care plans for families.
• Connect families with community resources and navigation supports.
• Assess eligibility for services and coordinate referrals for health, developmental, and social needs.
• Collaborate with service providers on developmental concerns and referral options.
• Follow up to monitor progress and ensure connections to services.
• Maintain accurate program data and improve service delivery with partners.
• Build relationships with community agencies for coordinated support.
