Solano Family & Children’s Services seeks a compassionate Family Navigator I to serve as the first contact for the Help Me Grow Solano program. You will assess needs, connect families to services, and document progress in the client records with attention to privacy and timelines.

The role emphasizes strong communication, organization, and problem-solving, with opportunities to support families and participate in outreach events in Solano County.

Qualifications

• Proven ability to manage multiple tasks in a fast-paced environment.
• Strong documentation and data entry skills.
• Comfort with learning new software and navigating databases.

Responsibilities

• Provide case management services, monitor referrals, develop family support plans, document case notes, and follow up within timelines.
• Assess family needs and coordinate referrals to community resources and developmental services.
• Maintain accurate client records, data entry, tracking, and reporting in compliance with procedures.
• Respond promptly and professionally to inquiries and referrals.
• Promote collaboration with service providers and community partners.
