Job summary

CVS Health is hiring an RN Case Manager for a fully remote role. You will assess, plan, implement, and coordinate case management for members, using clinical judgment and data review to reduce risks and improve wellness. Requires active RN license, 3+ years in acute care, and 1+ year in case management with CCM preferred.

Compact licensure allowed; strong communication and Office 365 skills required. Work hours are Mon–Fri 8:30am–5:00pm in the time zone of operations; some on-call, weekends, and

Qualifications

• Active, current, and unrestricted RN license in state of residence
• Compact RN licensure allowed if applicable
• 3+ years of acute care RN experience
• 1+ years of case management experience
• Certified Case Manager (CCM) certification preferred
• Associates Degree in Nursing required; Bachelor's preferred
• Strong customer service skills and proactive issue resolution
• Experience with Microsoft Office (PowerPoint, Excel, Word)

Responsibilities

• Collaboratively develops a proactive plan of action to improve short- and long-term outcomes and overall wellness for members
• Uses clinical tools and data review to apply clinical judgment and address health issues
• Follows case management processes per regulations and company policies
• Uses interviewing skills to engage members and assess health status and needs
