The position supports multiple locations with remote capabilities and travel as needed within the assigned area. Excellent communication and care coordination skills are essential for success.

Responsibilities

• Perform telephonic outreach for recently discharged and high‑risk patients.
• Facilitate discharge coordination and identify optional cost‑effective services.
• Consult with physicians, providers and members to assess, plan, implement, coordinate and monitor care.
• Serve as liaison to physicians and members and provide telephonic support across locations.
