Elevance Health in New York is seeking a Nurse Case Manager II to perform care management for members with complex needs, assessing, developing, implementing, coordinating, and evaluating care plans across the continuum, telephonically or on-site during discharge planning. The role supports collaboration with Medical Directors and Physicians, coordinates resources, and negotiates referrals within benefits.

Eligible candidates hold a BA/BS in a health-related field, an active RN license, and

Qualifications

• Requires BA/BS in a health-related field and minimum of 5 years of clinical experience; or equivalent experience.
• Current, unrestricted RN license in applicable state(s) required.
• Multi-state licensure required if providing services in multiple states.

Responsabilités

• Ensures access to services and develops a care management plan to address identified objectives and goals.
• Facilitates authorizations/referrals within benefits structure or through extra-contractual arrangements.
• Coordinates internal and external resources to meet identified needs.
• Monitors and evaluates the care management plan and updates as needed.
• Interacts with Medical Directors and Physician Advisors on care plans.
