Job summary

Univera Healthcare in Buffalo, NY seeks a Supervisor of Case Management to lead a dynamic team of Dietitians and Respiratory Therapists, driving innovation in care delivery and operational excellence. You will supervise CM staff, coordinate daily activities, participate in recruiting and training, and act as a CM liaison in internal and external meetings to improve health plan

Qualifications

• Current New York State clinical licensure (RN, Licensed Behavioral Health Clinician, PT, NP, PA, or Dietician) with a minimum of three years of case management/quality improvement experience or equivalent.
• Knowledge of NCQA and/or Medicare and Medicaid guidelines preferred.
• Ability to multitask and balance priorities.
• Proficient in computer use (Word, Excel, Internet, email).
• Strong leadership, communication, problem solving and analytical skills to interact with all levels of personnel.

Responsibilities

• Provides direction and supervision to the CM staff including departmental function, development plans, performance reviews, and disciplinary actions.
• Coordinates communications and daily activities of staff to meet Health Plan goals and ensures adequate staffing.
• Participates in recruitment, training, and retention of staff with license verification.
• Conducts periodic case reviews and staff performance evaluations to identify improvement opportunities.
• Maintains staff accountability for performance metrics and implements performance management as needed.
• May manage caseloads and lead CM programs related to physical and behavioral health.
