Capital District Physicians Health Plan Inc. in New York seeks a Supervisor of Case Management to supervise daily CM staff, coordinate department projects, staffing, and meetings, and represent CM at internal and external meetings.

You will lead a dynamic team including Dietitians and Respiratory Therapists while driving care delivery innovation and operational excellence, implementing initiatives that shape the organization’s future.

Qualifications

• Current New York State clinical licensure (RN, Licensed Behavioral Health Clinician, PT, NP, PA, or Dietitian) with relevant CM/quality improvement experience.
• Knowledge of NCQA and/or Medicare and Medicaid guidelines preferred.
• Ability to multitask and balance priorities.
• Proficient computer skills: Word, Excel, Internet and email.
• Strong leadership, written and verbal communication, problem solving and analytical abilities.

Responsibilities

• Provide direction and supervision to CM staff including department function and performance reviews.
• Coordinate communications and daily activities of staff and assure adequate staffing to meet goals.
• Participate in recruitment, training and retention of staff; verify licenses as applicable.
• Conduct periodic case reviews and staffing pattern assessments, drive improvements.
• Maintain staff performance metrics and implement performance management processes.
• May manage a subset of member CM programs (physical and behavioral health).
• Lead regular team meetings and maintain staff files with annual reviews and education records.
• Assist in policy/procedure development for CM and implement departmental changes.
• Develop and monitor new initiatives using project management skills.
• Provide presentations related to department functions and serve as liaison with other departments.
• Identify strategies to improve health care resource management and communicate results.
• Maintain documentation and prepare reports, including Quality Improvement activities.
• Ensure compliance with regulatory/accreditation standards and privacy policies.
