The role focuses on people leadership, workflow development, and implementing initiatives that enhance care delivery and operations.

The position requires NY state licensure and 3+ years in case management with strong leadership, communication, and analytical skills. Remote work may be considered case-by-case.

Qualifications

• Current New York State clinical licensure (RN, Licensed Behavioral Health Clinician, PT, NP, PA, or Dietitian) with a minimum of three years of case management/quality improvement experience or equivalent.
• Knowledge of NCQA and/or Medicare and Medicaid guidelines preferred.
• Must possess strong leadership and communication skills, plus problem solving and analytical abilities.

Responsibilities

• Provides direction and supervision to the CM staff including department function, development plans, performance reviews, and disciplinary actions.
• Coordinates communications and daily activities of staff and ensures adequate staffing to meet Health Plan goals.
• Participates in recruitment, training, and retention of staff with license verification when applicable.
• Conducts periodic staff and case reviews, identifies areas for improvement and initiates corrective actions including productivity monitoring.
• Holds staff accountable for performance metrics and manages performance processes.
