Job summary

Molina Healthcare is seeking a remote Care Transition Nurse/Coach to support hospital discharge planning and post-discharge transitions for members across the United States. The role emphasizes reducing readmissions and coordinating care in the home and community settings.

Responsibilities include overseeing a 30-day transition program, collaborating with discharge planners, and educating members on medication management, follow-up care, and community resources.

Qualifications

• At least 2 years experience in health care, with at least 1 year in discharge planning, care management or behavioral health setting.
• Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) if required by state contract/regulation.
• Valid driver’s license with reliable transportation for job-related travel.

Responsibilities

• Follow a 30-day care transition program from hospital admission through transitions to other settings to reduce readmissions.
• Collaborate with hospital discharge planners, hospitalists, outpatient providers, facility staff and family/support network.
• Ensure safe transitions with adequate caregiving, medical oversight and medication management.
• Coordinate with other providers and services to arrange necessary services and equipment for safe transition.
• Conduct face-to-face visits in hospital and post-discharge home visits for high-risk members as needed.
• Educate and support members on transition care pillars and follow-up care.
