Qualifications

• RN license active and unrestricted in state of practice.
• At least 2 years in healthcare with 1 year in discharge planning, care management or behavioral health.
• Experience with Care Transitions Intervention (CTI) or similar model.
• Ability to work across multiple settings and diverse populations.
• Strong communication, time management and self-motivation.

Responsibilities

• Support care transition activities from hospital admission through post-discharge.
• Coordinate with discharge planners, hospitalists, providers, and families.
• Ensure safe transitions with appropriate caregiving and medical oversight.
• Facilitate ICT meetings and collaborate with interdisciplinary teams.
• Travel 40-50% as required per state/contractual requirements.
