Job summary

Independence Blue Cross is seeking a Care Transition Specialist to support members moving from inpatient care to home or other settings. You will engage post-discharge, educate on medications, and coordinate follow-up with providers to reduce readmissions.

Work is fully remote within the Tri-State Area (Delaware, New Jersey, Pennsylvania). You will collaborate with nurses, social workers, and the care team to guide members through plan benefits and resources while documenting Outreach outcomes

Qualifications

• Experience in healthcare member outreach or case management.
• Knowledge of discharge planning and managed care principles.
• Ability to document interactions in health plan systems.
• Excellent written and verbal communication skills.

Responsibilities

• Member Outreach & Engagement: Contact members post-discharge to review instructions and needs.
• Educate members on medication adherence, symptom monitoring, and follow-up appointments.
• Identify social determinants of health barriers and connect to resources.
• Assist with understanding plan benefits and connect to member services.
• Care Coordination: schedule follow-ups with network providers and referrals.
• Monitoring & Documentation: document outreach outcomes in care management systems.
