Job summary

The ALS Association is seeking a remote Care Services Manager to develop professional and community relationships, ensuring high-quality care for people living with ALS and their families. You will manage care services programs, supervise staff in an assigned territory, and facilitate education and navigation assistance.

Responsibilities include coordinating referrals, evaluating support groups, planning outcomes with leadership, and maintaining documented engagements per standards.

Qualifications

• Bachelor’s Degree or related experience in social work, case management, health administration, allied health, nursing required.
• Minimum 4 years of related professional experience and/or training.
• Strong management skills: problem-solving, planning, and performance improvement.
• Knowledge of ALS is a plus, but not required.
• Experience with progressive neurodegenerative disease.
• Knowledge of Medicare/Medicaid, patient assessments, planning and case management preferred.

Responsibilities

• Conducts specialized care consultations and needs assessments for people living with ALS, caregivers, and families.
• Manage a portfolio of clients and supervise field staff to ensure services are delivered.
• Provide information on programs, services, resources and educational materials.
• Coordinate referrals to healthcare providers and community agencies.
• Evaluate and support the effectiveness of ALS support groups.
• Plan program outcomes and evaluation in collaboration with leadership.
• Engage with ALS individuals and caregivers on a regular schedule.
• Document engagements per Care Services Standards.
• Educate public and professionals about ALS and the Association’s services.
• Serve as liaison between ALS Clinic teams and the Association.
• Provide in-service education to staff and healthcare professionals.
• Participate in continuing education and mentorship for new staff.
