Job summary

Vivo Care, Inc. is hiring a healthcare professional to support remote patient monitoring and wellness coaching from a centralized remote setting.

The role focuses on building trust with patients, triaging vital signs, and guiding care teams toward patient-centered goals. The ideal candidate has 1–3 years of clinical experience and a valid RN/LPN/LVN license in a compact state, with the ability to work MST hours.

Qualifications

• Unrestricted RN/LPN/LVN license in a compact state.
• 1-3 years of clinical experience, Medicare and remote patient monitoring preferred.
• Strong communication skills and ability to work independently.
• Willingness to work MST hours.

Responsibilities

• Build relationships with patients using motivational communication techniques.
• Triage vital signs and collaborate with care teams to set patient-centric health goals.
• Manage physiological markers and patient messaging through a remote care platform.
• Perform monthly wellness calls and provide preventive health education and coaching.
• Meet team goals and follow escalation pathways for urgent care needs.
