Job summary

Cambia Health is seeking remote Care Management RNs to provide case, disease, and care coordination from home within Oregon, Washington, Idaho or Utah. This role supports a person-focused health care experience with strong clinical judgment and collaboration across teams.

Ideal candidates have an RN license, 3+ years clinical or case-management experience, and eligibility for licensure in ID, OR, UT, and WA.

Qualifications

• RN license in a U.S. state or territory; independent assessments within scope
• Minimum 3 years of direct clinical care or case-management related experience
• Active, unrestricted licensure; eligibility for licensure in ID, OR, UT, and WA

Responsibilities

• Conducts case management activities: assessment, planning, implementation, coordination, monitoring, and evaluation
• Apply clinical judgment to ensure compliance with medical policy and guidelines
• Collaborates with physicians, internal/external customers to resolve issues and improve quality of care
• Responds to inquiries professionally while protecting confidentiality
• Provides accurate documentation ensuring compliance with standards and timelines
• Coordinate resources and prioritize assignments to meet goals and timelines
• Monitor effectiveness of care plans and adjust as needed
