Qualifications

• RN license required for medical care management and active licensure in relevant states.
• Minimum 3 years of direct clinical care or case management experience.
• Eligible for licensure in Idaho, Oregon, Utah and Washington.

Responsibilities

• Conduct case management activities across assessment, planning, coordination, monitoring and evaluation.
• Apply clinical judgment to ensure compliance with medical policies and standards of care.
• Collaborate with physicians and other stakeholders to resolve care or claims issues.
• Serve as a resource, maintaining confidentiality and professional communication.
• Provide accurate documentation aligned with performance standards and timelines.
• Coordinate resources and prioritize assignments to meet goals.
