Vigilance Health, a REMOTE, full-time opportunity in the United States, seeks a Care Manager to build trust with patients over the phone and drive meaningful health outcomes. You will manage a patient panel, document progress, and coordinate with care teams to improve PCP visits, adherence, and overall health outcomes for diverse populations.

You Can Build Trust With a Stranger in the First 90 Seconds of a Call.
You know how to pick up the phone, connect with a patient you've never met, and get them talking — really talking — about their health. You listen for what's actually going on with them, and you know how to turn that conversation into trust, a plan, and a next step the patient actually follows through on.
Now imagine doing that with the full weight of an organization behind you — one built entirely around the belief that a phone call, done right, can change the course of someone's health. That's this role.
Vigilance Health partners with FQHCs, ACOs, IPAs, and payers to deliver Population Health and Care Management services that actually move the needle on patients’ health. We run Care Management, APCM, Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs — and we measure success the way our partners do: quality metrics, patient engagement, patient activation, and outcomes that show up in data.
Our purpose is simple, and we mean it: Change People’s Lives — no matter their circumstances.
The Care Manager is the heart of Vigilance. You’ll lead the charge in building meaningful relationships with patients — driving better health outcomes and empowering people to take control of their own healthcare. You’ll build trust, rapport, and accountability using the principles of Motivational Interviewing and clinical best practices, call after call, patient after patient.
Above all else, Care Managers are driven by an unyielding passion to make a difference in people’s lives. That passion shows up in the work itself — the volume of outreach you put in, the rapport you build, and your commitment to the kind of engagement, trust, and support that leads to real, long-lasting behavior change.
You’ll drive a high volume of outbound calls every day — ramping from roughly 45 calls and 5–20 patient connections a day in your first month to 75+ calls and 22+ connections by month three — because consistent outreach is what makes everything else possible.
You’ll build genuine rapport with your assigned patient panel using Motivational Interviewing techniques, turning first conversations into ongoing relationships patients trust and stay engaged with.
You’ll understand the risk stratification of your assigned panel inside and out, grow your “billable” and “at risk” patient panels, and work to minimize the “fallen out” panel — keeping patients engaged before they disengage.
You’ll set PCP visit SMART goals with appropriate patients and drive tangible improvements in health outcomes — PCP visits, medication adherence, care navigation, baseline assessments and more — through consistent, engaged care management.
You’ll translate every conversation into clear action and solid documentation, applying clinical best practices so the story of each patient’s status and progress are captured accurately and can be acted on by the rest of the care team.
You're charismatic and high-energy, with a pleasant voice and real vocal variety — the kind of person patients want to keep talking to. You're patient, resilient, and encouraging, with a coaching attitude and a great ear for what people aren't quite saying. You know how to smile and dial, you stay organized under a high volume of calls, and you bring a proactive, problem-solving mentality and a high degree of empathy to every conversation.
If you've been looking for a role where your ability to connect with people genuinely changes someone's health — this is it.
