Molina Healthcare is seeking a remote field-based Care Manager to coordinate LTSS services in the greater Cook County area. You will work with a multidisciplinary team to implement care plans, monitor progress, and address barriers with members, caregivers, and providers.

You will travel locally to perform home visits, complete assessments, and facilitate waiver processes while ensuring regulatory timelines and quality outcomes. Strong communication and license credentials are essential.

Formación

• At least 2 years in health care with 1 year in care management or managed care.
• Active professional license as listed (LCSW, LMSW, APSW, LPC, LPCC, LMFT, PhD/PsyD equivalent).
• In some states, a health-related bachelor's degree may be required.
• Valid driver's license, reliable transportation and adequate auto insurance for travel.
• Proactive, detail-oriented, and able to work independently.
• Experience with LTSS, disability populations, and behavioral health settings.

Responsabilidades

• Supports care management/care coordination for LTSS across the continuum.
• Collaborates with multidisciplinary teams to coordinate member care.
• Develops and implements care plans with member and care network.
• Monitors care plans, documents interventions and outcomes, adjusts as needed.
• Promotes service integration including behavioral health and LTSS.
• Assesses medical necessity and authorizes waiver services.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing to educate and motivate members.
• Identifies barriers and coordinates psycho/social support and resources.
• Travel up to 25-40% locally as required by state/contract.
