Job summary

Molina Healthcare is seeking a field-based care coordination professional to support LTSS delivery. You will perform home visits, coordinate services with a multidisciplinary team, and develop comprehensive care plans to meet member goals.

Travel will be required to Marion and Mahaska Counties, with a focus on quality and cost-effective member care. Strong communication and organizational skills are essential for success.

Qualifications

• 2+ years of health care experience with LTSS and care management.
• Licensure/certification required where applicable by state contract.
• Valid driver's license and reliable transportation for travel.
• Experience with diverse populations and community resources.

Responsibilities

• Provide LTSS care management and coordination across settings.
• Perform comprehensive member assessments within timelines.
• Develop and monitor care plans with the care team and member network.
• Promote integration of behavioral health and LTSS services.
• Assist with waiver enrollment/disenrollment and funding decisions.
• Facilitate ICT meetings for service approvals/denials.
• Travel 25–40% to member homes in Marion/Mahaska counties.
