Job summary

Qualifications

• Master's degree in health, psychology, social work or nursing.
• Licensure as RN/LCSW/LCMHC or related clinical credential preferred.
• Experience with Medicaid and NC programs is preferred.

Responsibilities

• Perform health risk assessments and develop person-centered care plans.
• Coordinate services across MH/SU/IDD/TBI and medical domains.
• Collaborate with an interdiscliplinary team to ensure appropriate care.
• Maintain HIPAA compliance and accurate documentation.
