Job summary

Medix is seeking a patient-centered Remote Care Manager to engage Medicaid members by phone, identify social determinants of health, coordinate resources, and develop tailored care plans addressing barriers to care. In a collaborative virtual setting, you will document interactions in the EHR, monitor outcomes, and support members through referrals to community services and healthcare providers.

Qualifications

• Experience working with Medicaid populations, managed care organizations, or community health environments strongly preferred.
• Core Competencies: Address SDOH and navigating local social services.
• Communication Skills: Telephonic communication with motivational interviewing and culturally competent care.
• Technical Skills: Familiarity with EHR and care management software; able to work remotely.
• Attributes: Empathy, problem-solving, self-motivation, attention to detail.

Responsibilities

• Conduct outbound phone outreach to assess SDOH.
• Develop and implement member-centric care plans.
• Coordinate referrals to community-based organizations and providers.
• Follow up to ensure service connections and address barriers.
• Document interactions and outcomes in EHR in a timely manner.
• Monitor KPIs and report referral outcomes.
