Job summary

Medix is hiring Remote Care Managers to support Medicaid members in navigating healthcare and social resources. This role emphasizes outreach, assessments, and personalized care planning in a fully remote setting.

You’ll work with a team to coordinate referrals, document interactions in care-management systems, and help members overcome barriers to care, with a Monday–Friday schedule and a company-issued laptop. The target start date is October 26, 2026.

Qualifications

• Minimum 1 year of experience in care management or related field.
• Experience supporting Medicaid members or high‑risk populations preferred.
• Knowledge of social determinants of health and community-resource navigation.
• Experience conducting assessments and coordinating referrals or services.
• Strong documentation, organization, and time-management skills.
• Comfortable with outbound outreach and persistent follow-up with members.
• Ability to work independently in a remote environment.
• High school diploma or GED required.

Responsibilities

• Conduct outbound outreach to Medicaid members by phone and other approved communication methods.
• Complete assessments to identify healthcare and social needs.
• Develop individualized, member-centered care plans based on identified needs and goals.
• Help members navigate resources related to housing, food insecurity, transportation, financial assistance, healthcare access, benefits, behavioral health, and other social determinants of health.
• Coordinate referrals with healthcare providers, community organizations, and social-service agencies.
• Follow referrals through completion and confirm members successfully connect with needed services.
• Provide ongoing education, support, and follow-up to members.
• Use motivational interviewing and person-centered communication to encourage engagement.
• Document assessments, member interactions, referrals, care plans, and outcomes in care-management systems.
• Collaborate with internal teams, healthcare providers, and community partners.
• Manage outreach, documentation, follow-up, and productivity expectations in a structured remote environment.

Pay: $21–$24/hour

Schedule: Monday–Friday | 9:00 AM–5:30 PM ET

This is a strong opportunity for someone with experience in case management, care coordination, behavioral health, community health, social services, or Medicaid programs who enjoys working directly with members and helping them navigate healthcare and community resources.

What We’re Looking For

• At least 1 year of experience in care management, case management, care coordination, community health, behavioral health, social services, or a related field
• Experience supporting Medicaid members or high-risk and underserved populations strongly preferred
• Knowledge of social determinants of health and community-resource navigation
• Experience conducting assessments and coordinating referrals or services
• Comfortable with frequent outbound phone outreach and persistent follow-up with members
• Strong documentation, organization, and time-management skills
• Comfortable navigating multiple healthcare, EHR, or care-management systems
• Strong communication skills, empathy, persistence, and follow-through
• Ability to work independently in a remote, productivity-focused environment
• High school diploma or GED required
