Job summary

Molina Healthcare is seeking a Care Manager to support UT Medicaid members recently admitted to hospital. This telephonic role requires strong computer skills, attention to detail, and the ability to multitask across systems while documenting accurate contact notes.

Travel to designated hospitals within the local service area is reimbursed, with no weekends or nights. Preferred candidates have background in case management or managed care; RN background is a plus.

Qualifications

• At least 2 years healthcare experience, preferably in behavioral health.
• Licensed behavioral health clinician with active, unrestricted license.
• Valid driver’s license and reliable transportation.
• Experience with severe and persistent mental health concerns and substance use.
• Data entry on clinical platforms.
• Excellent verbal and written communication.
• Proficiency with Microsoft Office.

Responsibilities

• Completes comprehensive behavioral health assessments within regulated timelines and determines eligibility for care coordination/case management.
• Develops and implements care plans with member, caregiver, physician and/or other healthcare professionals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and goal achievement.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services to enhance continuity of care.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and coordinates resources for members.
• May provide consultation to peers as needed.
• 25-40% estimated local travel may be required.
