Job summary

Centene Corporation is seeking a Care Manager to develop and facilitate complex care management activities for members with mental and behavioral health needs. You will assess needs, create personalized care plans, and coordinate with providers and families to ensure timely access to services.

The role supports foster care members, involves up to 75% local travel, and offers remote work for the remaining time. A sign-on bonus of $2,500 is available, with a Monday–Friday schedule.

Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or Nursing.
• Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN per state contract requirements.

Responsibilities

• Evaluate member needs via phone or in-home visits and develop care/service plans.
• Coordinate between members, families, and care teams to ensure access to services.
• Develop ongoing care plans for high-acuity members and identify providers/resources.
• Monitor plans and adjust based on symptoms and side effects.
• Educate members and families on procedures, options, and benefits.
