Job summary

Centene Corporation in Kansas is seeking a Behavioral Health Care Manager to develop and coordinate complex care plans for members with mental health and substance use needs. The role emphasizes education for members and families, collaboration with providers, and ensuring access to services.

You will travel up to 75% within NW Kansas counties, with remote work for the remainder, Monday–Friday schedule, and a $5,000 sign-on bonus.

Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or a Nursing degree with BH experience.
• 2–4 years of related experience in behavioral health care management.
• Licensed professional (e.g., LCSW/LMSW/LMFT/LMHC/LPC or RN) as per state contract requirements.

Responsibilities

• Evaluates member needs via phone or in-home visits and facilitates care plans for behavioral health and social determinants.
• Performs outreach to assess needs and coordinates with resources.
• Develops care plans for high-acuity members and identifies providers and community resources.
• Coordinates between member/family and care providers to ensure access to services.
• Monitors plans and outcomes, adjusting as needed based on side effects and symptoms.
• Facilitates care coordination and timely access to needed care.
• Collects and maintains member information to ensure regulatory compliance.
• Educates members and families on procedures, instructions, referrals, and benefits.
• Provides leadership with feedback to improve cost-effective care delivery.
• Performs other duties as assigned and complies with policies.

Education

Master's degree in Behavioral Health or Social Work

Nursing degree from an Accredited School of Nursing

Licensed Master's Behavioral Health Professional or RN license
