Job summary

Astrana Health, Inc. is seeking a Supervisor - Care Management (Non-clinical) to lead daily supervision of care management teams, including Community Health Workers and Care Coordinators, across North East market populations.

The role ensures high-quality, cost-effective, patient-centered care through strong leadership and policy adherence. This remote position reports to the Senior Manager, Care Management.

Qualifications

• High school diploma; diploma/certificate in management preferred.
• At least 2 years of case/care management experience in a medical group, IPA and/or HMO setting preferred.

Responsibilities

• Supervise care management teams daily, ensuring adherence to ambulatory care management policies, procedures, and regulatory requirements.
• Oversee delivery of care management services across the continuum, supporting staff performance and clinical excellence.
• Ensure consistent application of policies and take corrective or disciplinary actions when necessary.
• Other projects and duties as assigned by Clinical Leadership team.
• Provide coaching and performance evaluations for staff and oversee hiring, onboarding, and training.
