Job summary

Centene Corporation is seeking a remote Care Coordination Supervisor for long-term care, supporting a small Iowa territory with some travel. You will supervise the team, manage escalations, and ensure compliant, high-quality care delivery across members and providers.

You will monitor documentation, assign caseloads, mentor staff, and drive onboarding and staff development while coordinating with senior management to address complex cases.

Qualifications

• Requires a Bachelor’s degree and 4+ years of related experience.
• Iowa-specific: Bachelor’s degree + 4+ years with populations served, or RN with 6+ years with populations served.
• Equivalent experience through applicable knowledge, duties, scope and skill.

Responsibilities

• Supervises the care coordination team in long-term care to promote quality and efficacy of care management delivery.
• Assigns caseloads and work assignments to staff based on state requirements and member needs.
• Addresses escalated and complex care cases with guidance from senior management.
• Educates and provides resources for the team to facilitate ongoing communication with members and providers.
• Evaluates team performance and provides feedback on goals and career milestones.
• Coaches and guides the team to improve member and provider experience and care delivery.
• Assists with onboarding, hiring, and training of team members.
• Champions change within scope of responsibility.
• Performs other duties and complies with policies.
