Job summary

PacificSource is hiring a Care Management Clinician in Boise, Idaho. This full-time role focuses on advocacy, assessment, planning, and coordination of care to improve clients’ health outcomes and optimize plan benefits.

The Clinician will collaborate with providers, payers, and employers while maintaining patient privacy and regulatory compliance. Ideal candidates have a minimum of three years in clinical case management and hold an RN license or equivalent licensure (LPC/LMFT/LCSW) with CCM

Qualifications

• Active, unrestricted RN license or licensed mental health professional credential (LPC/LMFT/LCSW) required.
• CCM certification required at time of hire or within two years.

Responsibilities

• Coordinate and deliver case management services to clients to promote wellness and autonomy.
• Assess patient information and develop individualized care plans.
• Link clients with providers and resources across the care continuum.
• Maintain patient privacy and comply with HIPAA and regulatory standards.
• Collaborate with payers, primary care providers, and other service delivery professionals.
