Qualifications

• Associate or Bachelor’s Degree in Nursing or related field with 3 years of case management experience.
• Active US-state licensure/certification allowing independent assessment and at least 3 years of direct clinical care.
• URAC case manager certification or related health degree/licensure (RN or behavioral health) as applicable.

Responsibilities

• Perform essential case management activities including assessment, planning, implementation, coordination, monitoring, and evaluation.
• Ensure compliance with medical policy and standards within licensed scope.
• Collaborate with physicians and internal teams to resolve claims and quality issues.
• Document interactions professionally and maintain patient confidentiality.
• Communicate with members and providers timely and accurately.
