Qualifications

• Bachelor's degree with two years of relevant experience required.
• Master’s degree with one year of relevant experience preferred, or RN with two years of relevant experience.
• Demonstrated ability to work with diverse populations and multidisciplinary teams.

Responsibilities

• Coordinate enrollee services and develop care plans to ensure comprehensive care.
• Communicate with caregivers and service providers to facilitate access and adherence to treatment.
• Maintain documentation using care coordinator portal and EHR systems.
• Attend care planning meetings and coordinate referrals and follow-up.
• Travel to assigned operational areas and facilities on Long Island as needed.
