Medica is seeking Care Coordinators to support members across Minnesota from a remote home-based setting. You will develop individualized care plans, coordinate medical and social services, and serve as a key contact for questions about Medicare/Medicaid benefits.

The role emphasizes independence, strong communication, and collaboration with clinicians to optimize member outcomes. Travel to member homes is required within Medica's service area, with mileage reimbursement provided.

Formación

• RN license or degree in nursing with required state credentials.
• Experience in community based services preferred.
• Proven ability to coordinate care and document effectively.

Responsabilidades

• Manage a caseload of members and coordinate delivery of medical and social services.
• Educate members about benefits and act as primary contact.
• Collaborate with physicians and providers to discuss care plans.
