Job summary

Magellan Health is seeking a Care Coordinator to manage and coordinate care for members in the Las Cruces area, with remote/work-from-home flexibility and travel as required. You will conduct assessments, develop care plans, coordinate services and monitor outcomes in collaboration with an interdisciplinary team.

The role requires 3-5 years in social work, nursing, or healthcare, with relevant qualifications and licensure.

Qualifications

• 3-5 years' experience in Social Work, Nursing, or Healthcare-related field.
• Experience in utilization management, quality assurance, home or facility care, community health, long term care or occupational health required.
• Strong communication and analytical skills; ability to work with clinicians, hospital staff and community partners.

Responsibilities

• Provide care coordination to members with behavioral health conditions requiring intensive interventions and oversight.
• Conduct health risk assessments and comprehensive needs assessments including psycho-social, medical and environmental factors.
• Develop and monitor care plans; serve as primary contact to ensure services are delivered appropriately.
• Coordinate and monitor strategies to improve health outcomes and quality of life.
• Advocate for member care needs and address gaps in care.
• Collaborate with interdisciplinary team to adjust plans as needed and evaluate effectiveness.
