Job summary

Magellan Health is seeking a CISC Care Coordinator to coordinate services for members with behavioral health needs. You will perform risk assessments, develop and execute care plans, and liaise with a multidisciplinary team to ensure quality outcomes.

The role emphasizes advocacy, resource coordination, and continuous care evaluation. The position supports virtual or face-to-face work based on contractual requirements, with a focus on cost-effective, high-quality care and member satisfaction.

Qualifications

• 3-5 years' experience in Social Work, Nursing, or Healthcare-related field, or relevant experience in lieu of degree.
• Experience in utilization management, quality assurance, home or facility care, community health, long term care or occupational health required.
• Knowledge of cost/benefit analysis, negotiation, and cost containment.
• Ability to interpret data and make decisions with analysis.
• Strong written and verbal communication with clinicians, hospital officials and service agency contacts.

Responsibilities

• Provides care coordination to members with behavioral health conditions identified and assessed as requiring intensive interventions and oversight.
• Conducts in depth health risk assessment and/or comprehensive needs assessment including psycho-social, physical, medical, behavioral, environmental, and financial parameters.
• Communicates and develops the care plan and serves as point of contact to ensure services are rendered appropriately.
• Implements, coordinates, and monitors strategies for members and families to improve health and quality of life outcomes.
• Develops, documents and implements plan with resources to address social, physical, mental, emotional, spiritual and supportive needs.
• Acts as an advocate for member's care needs and addresses gaps in care.
• Monitors plan of care effectiveness and adjusts as needed with interdisciplinary team.
