Job summary

Magellan Health, Inc. seeks a CISC Care Coordinator to deliver care coordination for individuals with behavioral health needs, in a work-from-home role with travel across the Albuquerque area and beyond as required by contracts.

You will assess needs, develop care plans, coordinate services, and monitor outcomes to improve quality and cost-effectiveness. Working with interdisciplinary teams, you will educate providers and families, advocate for members, and ensure timely access to community

Qualifications

• 3-5 years' experience in Social Work, Nursing, or Healthcare-related field or relevant substitute in lieu of degree.
• Experience in utilization management, quality assurance, home or facility care, community health, long term care or occupational health.
• Ability to analyze trends using decision support systems.
• Strong business acumen including cost/benefit analysis, negotiation, and cost containment.
• Knowledge of referral coordination to community and private/public resources.
• Ability to make decisions requiring significant analysis and investigation with solutions requiring original thinking.
• Ability to determine appropriate courses of action in more complex situations beyond existing policies.
• Ability to maintain complete and accurate enrollee records.

Responsibilities
