Magellan Health is seeking a CISC Care Coordinator for a work-from-home role serving Las Cruces, NM and surrounding areas with extensive community travel. You will coordinate care for individuals with behavioral health needs, assess risks, develop and monitor care plans, and collaborate with interdisciplinary teams to improve outcomes.

The role emphasizes cost-effective, quality care, education of providers and families, and advocacy for members.

Formación

• 3-5 years' experience in Social Work, Nursing, or Healthcare-related field.
• Experience in utilization management, quality assurance, home or facility care, community health, long-term care or occupational health.
• Ability to analyze trends based on decision support systems.
• Knowledge of cost/benefit analysis, negotiation, and cost containment.
• Understanding of care coordination and interdisciplinary teams.

Responsabilidades

• Provides care coordination to members with behavioral health conditions requiring intensive interventions and oversight.
• Conducts in depth health risk assessment and comprehensive needs assessment.
• Develops and communicates the care plan; serves as point of contact for services.
• Implements, coordinates, and monitors strategies to improve health and quality of life outcomes.
• Advocates for member needs and identifies gaps in care; maintains enrollee records.
• Educates providers and staff about the care coordination role and health strategies.
• Collaborates with interdisciplinary care plan team to address needs across the continuum.
