Qualifications

• 3-5 years' experience in Social Work, Nursing, or Healthcare-related field.
• Experience in utilization management, quality assurance, home or facility care, community health, long term care or occupational health.
• Ability to analyze trends and interpret data to inform care decisions.
• Excellent verbal and written communication with clinicians and service partners.

Responsibilities

• Provide care coordination to members with behavioral health conditions requiring intensive interventions and oversight.
• Conduct health risk assessments and comprehensive needs assessments across psycho-social, physical and medical domains.
• Develop and monitor care plans; coordinate transition to home or community-based services.
• Educate providers, support staff, members and families about care coordination strategies and resources.
• Collaborate with interdisciplinary care plan teams to address gaps in care and improve outcomes.
