Job summary

Magellan Health is seeking a CISC Care Coordinator for Chavez and Eddy County, NM. The role is remote with extensive travel and requires residency in Roswell or nearby areas to coordinate community-based care.

You will assess needs, develop plans, monitor outcomes, and advocate for members across medical, behavioral health, and social services. Strong communication and healthcare know-how are essential for success.

Qualifications

• 3-5 years' experience in Social Work, Nursing, or healthcare-related field or relevant experience in lieu of degree.
• Experience in utilization management, quality assurance, home or facility care, community health, long term care or occupational health required.
• Business management skills including cost/benefit analysis, negotiation, cost containment.
• Knowledge of referral coordination to community and private/public resources.
• Ability to make decisions requiring significant analysis and investigation with solutions requiring original thinking.
• Ability to determine appropriate courses of action in more complex situations beyond existing policies.
• Ability to maintain complete and accurate enrollee records.
• Excellent verbal and written communication skills.

Responsibilities

• Coordinate care for members with diverse health needs across medical, behavioral health, and social services.
• Develop and monitor care plans, ensuring timely and cost-effective service delivery.
• Collaborate with interdisciplinary teams to address gaps in care.
• Educate providers and families about care coordination processes.
