Responsibilities

• Promote the appropriate use of clinical and financial resources to improve quality of care and member satisfaction.
• Provide care coordination across medical, behavioral health and social services.
• Educate providers and staff on care coordination roles and health strategies.
• Serve as liaison with interdisciplinary care plan team to address care issues.
