Magellan Health is seeking a Care Coordinator to support Eddy, Lea and Chaves County residents. The role involves planning, implementing, coordinating and evaluating care to achieve quality outcomes and cost-effective results.

You will work with an interdisciplinary team, educate providers and families, and advocate for members while monitoring the plan of care and ensuring services are delivered as contracted.

Formación

• 3–5 years' experience in Social Work, Nursing, or healthcare field.
• Experience in utilization management, quality assurance, home or facility care.
• Knowledge of cost-effective coordination of care and decision support.
• Strong verbal and written communication skills; ability to work with clinicians and officials.

Responsabilidades

• Coordinate care plans across interdisciplinary teams.
• Assess and monitor plan of care; adjust as needed.
• Educate providers and families about care strategies.
• Advocate for member needs and identify gaps in care.
