Job summary

Crisis Preparation and Recovery in Tempe is seeking a Program Care Coordinator for the Transitional Care Program. You will provide administrative, coordination, and operational support to ensure timely access to care, continuity of services, and effective communication among internal teams and external partners.

Qualifications

• High School Diploma or GED required.
• Minimum of 2 years of experience in behavioral health, healthcare administration, care coordination, or a related field.
• Associate's or Bachelor's degree in Social Work, Behavioral Health, or a related field preferred.
• Ability to qualify as a Behavioral Health Technician (BHT), if required by program.

Responsibilities

• Respond to phone calls, emails, and referrals in a timely and professional manner.
• Coordinate referrals, scheduling, and documentation in CPR's electronic medical record and tracking systems.
• Communicate with internal teams, providers, and external partners to support continuity of care.
• Conduct outreach to clients, referral sources, and community partners as required.
• Accurately collect, enter, and maintain client, referral, and service data.
• Monitor timelines and follow up to ensure program and contractual requirements are met.
• Identify and elevate concerns or barriers to supervisors as appropriate.
• Maintain confidentiality and comply with all HIPAA, CPR, and departmental policies.
• Provide cross-coverage and support within the department as needed.
• Reliable and predictable attendance is an essential job function.
• Complete other duties as assigned.
