Job summary

Wellthy is seeking a Care Coordinator to guide families across the United States through the caregiving process. You will manage logistics, build relationships, and develop comprehensive care plans, coordinating with care advisers to relieve families of administrative burdens while delivering high-quality service.

The role emphasizes proactive problem solving, efficiency, and strong collaboration with teams to support loved ones’ health and well-being.

Qualifications

• Bachelor’s degree required Social work, Public Health, Healthcare Administration or similar.
• 4-6+ years of healthcare experience, specifically case management, crisis intervention (or equivalent) and coordination experience.
• Knowledge of and ability to navigate the healthcare sector and communicate in a way that is empathetic, easy to understand and thorough.
• Experience developing and facilitating care plans.
• Ability to take constructive feedback and become innovative in the care plan approach that uniquely supports the member.
• Adaptable and able to work efficiently while managing up to 40 care projects with care and compassion.

Responsibilities

• Guide Wellthy families through caregiving complexities, managing logistics and priorities.
• Coordinate medical appointments, research providers, and track medications.
• Assist with insurance and facility vetting, and manage medical bills.
• Develop comprehensive care plans and oversee ongoing family needs.
• Collaborate with Care Advisers to ensure seamless, high-quality service.

Education

Bachelor’s degree in Social Work or related

Advanced degrees are a plus
