Job summary

Cardiac Study Center in Washington state seeks a Registered Nurse Navigator to bridge patients with cardiology care. This remote-first, full-time role supports referrals, access, outcomes, and education while leading coordination across care teams.

Qualifications

• Current WA State RN license (Multistate license required within 6 months of hire).
• Bachelor’s degree in Nursing required; BSN preferred.
• 3+ years of recent clinical experience in cardiology or related specialty.

Responsibilities

• Care Navigation & Patient Advocacy: coordinate referrals, access, and follow-ups; liaison between patients, providers, and care team; provide psychosocial support.
• Program Monitoring & Quality Improvement: collect and analyze outcomes data; identify best practices; support training and workflow improvements.
• Education & Community Engagement: educate patients and families; support staff training and create educational materials.
• Team Collaboration & Leadership: act as clinical expert; drive cross-team coordination and multidisciplinary meetings.
