Seamlessassist seeks a Remote Care Navigator for Cardiac to support medically complex patients with heart failure via phone outreach, care plan coordination, and CMS-compliant documentation. The role is non-clinical and works under RN supervision to reduce hospitalizations and improve self-management.

The position is full-time, 40 hours weekly, remote in the US (Dallas/Fort Worth area preferred). Requires MA credential, 2+ years in care coordination, and US work authorization.

Qualifications

• Active MA certification or equivalent clinical credential as listed.
• Minimum 2 years in care coordination, case management, or ambulatory care.
• Familiarity with CMS PCM/CCM/TCM requirements and documentation.
• Tech-savvy with care coordination software and EHRs.
• AI fluency and ability to use AI tools to improve efficiency.
• Authorized to work in the United States with US time-zone overlap.
• Excellent written and verbal English communication.

Responsibilities

• Conduct structured telephonic outreach to CHF and complex cardiac patients.
• Maintain a patient caseload using risk stratification for outreach.
• Complete assessments and follow-ups covering symptoms, meds, psychosocial status, and SDOH.
• Support TCM follow-up within 48 hours post-discharge with reconciliation and scheduling.
• Educate patients on CHF self-management and evidence-based strategies.
• Monitor for worsening signs and escalate to supervising RN as needed.
• Review population health dashboards and address care gaps.
• Document care plans and goals per CMS standards, coordinating with the care team.
