Job summary

Heartbeat Health is seeking a Nurse Practitioner for our virtual clinic serving the Eastern Time Zone. You will deliver high-quality cardiovascular care via telemedicine, develop treatment plans, and monitor patients with a focus on Heart Failure management and GDMT. Collaboration with cardiologists and primary care teams is essential.

Eligible candidates hold an MSN in cardiology, an NP license, and cardiology certification. Experience in outpatient cardiology and remote care is highly valued.

Qualifications

• MSN with cardiology specialization required.
• NP license in good standing, preferred compact state.
• Cardiology NP certification from ANCC or AANPCB preferred.

Responsibilities

• Conduct telehealth patient assessments including history and examinations.
• Formulate differential diagnoses and treatment plans for cardiac patients.
• Manage heart failure care pathways and GDMT titration.
• Collaborate with cardiologists and PCPs to coordinate care.
• Maintain accurate patient records and documentation.
