Job summary

Heartbeat Health, Inc. seeks a Nurse Practitioner to join our virtual cardiovascular care team, supporting Central Time Zone hours from 9 am to 5 pm CST, Monday through Friday.

You will assess and manage patients with heart failure and other cardiac conditions remotely, collaborate with cardiologists and PCPs, and guide GDMT, diagnostics, and education to improve outcomes. This role emphasizes compassionate, evidence-based practice in a remote-first environment, with emphasis on patient

Qualifications

• MSN with cardiology specialization and NP licensure.
• 3+ years in outpatient cardiology or similar settings.
• Experience delivering care via telemedicine.

Responsibilities

• Assess patients via telehealth, review history, exam, and tests.
• Formulate diagnoses and comprehensive treatment plans including GDMT.
• Independently manage heart failure and other cardiac conditions remotely.
• Educate patients on medication adherence, lifestyle, and GDMT titration.
• Collaborate with cardiologists, PCPs, and care team members.
• Document and maintain accurate patient records and discharge summaries.
